Student Absence Reporting Form

As you know, our Office of Student Services makes every effort to be consistently responsive to the needs of working professionals.  To aid in these efforts, we would like to know what conflicts arise that keep you away from the classroom.  Please make use our of this new online form to call in your absence for class. Simply fill out this form and tell us when and why you will miss class (sickness, family responsibilities, traveling for work, etc.).

As the H1N1 virus begins to spread, it is very important that you tell us if you are absent due to the Swine Flu.
